SMS Messaging Terms & Compliance

1. Program Description: This messaging program sends appointment-related text messages to customers who have booked mobile lab services with UptrendCTX through our website at https://uptrendctx.com or via our scheduling forms and have explicitly opted in to receive SMS notifications. Messages include appointment confirmations, reminders, rescheduling updates, technician arrival notifications, and customer support communications related to Live Scan Fingerprinting, DOT/Non-DOT Testing, and Mobile Phlebotomy services. No marketing or promotional messages are sent through this program.
2. Cancellation Instructions: You can cancel the SMS service at any time by texting "STOP" to the same number that sent you messages. Upon sending "STOP," you will receive a confirmation and no further SMS messages will be sent unless you re-opt in.
3. Support Information: For issues, reply "HELP" or contact support at (737) 361-8168 during business hours.
4. Carrier Liability: Carriers are not liable for delayed or undelivered messages.
5. Message & Data Rates: Message and data rates may apply. Frequency varies based on your appointment schedule, typically 2-4 messages per booked appointment. Contact your wireless provider for details.
6. Supported Carriers: Works with all major U.S. carriers, including AT&T, T-Mobile, Verizon, Sprint, and most regional carriers.
7. Age Restriction: You must be 18 years or older to participate in the SMS program.

Service Terms

UptrendCTX provides mobile laboratory services across Atlanta and surrounding Georgia areas, including but not limited to Gwinnett County, Johns Creek, Alpharetta, Snellville, Hoschton, and Loganville. Our services include Live Scan Fingerprinting, DOT and Non-DOT Testing, and Mobile Phlebotomy.

By booking an appointment, you agree to provide accurate identification and personal information as required for the specific service requested. Some services, such as fingerprinting and regulated drug testing, require submission of your information to authorized government agencies, employers, or regulatory bodies as part of the service.

Appointment cancellations or rescheduling requests should be made at least 24 hours in advance when possible. Same-day appointments are subject to availability.

General Terms

This website (the "Site") is owned and operated by UptrendCTX ("COMPANY," "we," or "us"). By using the Site, you agree to these Terms of Service, our Privacy Policy, and any additional terms applicable to specific sections or services.

Online Commerce
